Wow. This is a very interesting read!

My sense is there is significant selection bias though. The numbers seemed awfully skewed to the high end, at least for my region.

Yes. And payor mix is still king.

Even if your average commercial unit is $100 but your payor mix is 70% Medicare ($20/unit) you’ll end up with an average unit value of $44.

100x30=3000
20x70=1400
4400/100=$44unit

If you have a 50-50 payor mix with an average commercial unit of $70, you’d get $45/unit.

70x50=3500
20x50=1000
4500/100=$45/unit

In Florida, a 50/50 payor mix of Medicare/Commercial would be outstanding leaving you a $45-$50 blended unit. That said, very few hospitals besides the Mayo Clinic has that blended unit with most facilities at around $30-$35 per unit which requires a subsidy. Even ASCs have to give subsidies if they want to do a lot of CMS.
