Case western MSMP to medical school

undergrad sGPA 2.9 with 3.6 upward trend of last 20 sciences course. cumulative uGPA of 3.1.

3.7 gpa at case western msmp. 511 mcat. rural applicant, first generation in medicine. 500 hours as PCA at major cancer hospital. Everything else in order. What are my chances at different Ohio schools? Prefer MD.

Your best chances are Toledo, Wright State and NEOMED. You can try Ohio State and Cincinnati though your MCAT of 511 is less competitive for those schools. Also include West Virginia.
You should apply broadly to DO schools and I suggest these:
