We have an option at our school to take a year and get the Masters of Public Health degree. I am interested in health care and prevention. I am a 3rd year and will be doing this between 3rd and 4th year, taking my Step II either Aug or Sept of starting my 4th year.

Has anyone taken a year off or considered it? And what are the advantages/disadvantages?

There were a few people in my class who took time off between 2nd and 3rd year to pursue a PhD degree. I think that if your school offers financial assistance in obtaining a MPH degree and it is relevant to your future career plans, then it might be a worthwhile endeavour. However, I would encourage you to take Step 2 immediately after your third year, as the material would be freshest in your mind as opposed to coming back to take it after a year away from any clinical exposure.
