I'm struggling to decide whether to rank a program that has better reputation higher on my rank list or rank a program that's a community program but at my home town where I want to settle down in the future. I'm thinking of doing GI fellowship, but might also want to do primary care. I guess my real question is if I go into primary care, is it more important to stay local to make connections or go to a big program.

If you are going to stay strictly in primary care than I would say go where you are happiest and also can make some local connections. Remember, the attendings you train with will also create a referral base for you should you stay in the area.

However, if you plan to do a fellowship, especially one as competitive as GI, than you might be better off going to a slightly bigger name program.

In the end, you can do it either way and still make it work!
