how long does it take (on average) to get your medical license? do they send the form/license to you or to your institution?

As long as you provide all the documentation and fees, I don't recall it taking that long. Just the same, the earlier you start the process, the better, so you might want to obtain the paperwork if you don't already have it:

I have already filled it all out. My program sent it to me the Saturday after I found out where I matched. My only concern is that when I put my address on the form I had to put the address where I am currently residing. I just want to know that it will get here before I have to move to Michigan at the beginning of June.
