Need Pain/PM&R for patient moving to Tampa Bay area (winters Nov-June)

doc who does USGI and knee RFA (medicare)

Pt does not take opioids

Needs:

1) q 6M Synvisc ONE L knee ***USGI*** (b/c BMI and DJD)

2) L knee RFA (can be done office as he doesnt need MAC)
