How important are these? I understand and appreciate that programs are trying to show us a good time, take us somewhere nice, let us get a better feel for things, etc., but when your average Joe Six-pack is trying to avoid booking a hotel room by staying with friends/relatives who live 1-2 hours away, having to drive back and forth (rather than just once the morning of the interview) becomes somewhat impractical. Would missing a dinner adversely affect one's chances?

I feel it could hurt more than help if you act boorishly.

That being said, why turn down free food and a nice social hour?

Bribe your friends/relatives with some food/drink from the joint if they'll drive you. Stress deleted.

Since derm programs are small and less ppl will be at these than say prelims... I def think its worth ur time to go. Get the real story from the residents.. And hey if they really like you it can only help. The above does not apply if u are socially inept.
