Pre-Matriculation Certiphi Drug Screen Chain of Custody Form

Hello! When I went to do my pre-matriculation drug screen at the collection site Certiphi had me select, I found out that the specific site did not do drug screens and they had me go to another Certiphi-approved site. When I spoke with the Certiphi customer service staff over the phone, they told me to ask for the chain of custody form as a receipt at the new site.

I'm wondering if anyone has run into this issue before; do you just ask the lab tech for a copy of the chain of custody form? If not, do you get something else as the receipt?

Every time I've been drug tested I've received a COC form - I believe it is part of the regulations in place.

Thank you! I didn't realize COC was a regulatory requirement, and I thought I was asking for an internal document. The tech gave me the COC after the screen today, so it all worked out. Thank you!
