Do medical schools take leave of absence seriously and try to find out why the applying student took a LOA? (Like asking if the LOA was required, conditional or a health issue and seeing it negatively if they find out it was required)

There will often be questions in secondaries that ask you to explain any temporal gaps in your formal education. This would be the place to explain your reasons for a LOA.

Your health information is protected; med schools do not have the right to specifically ask you if you were ill, and if you merely say "LOA for health reasons", that's as far as they can inquire.

Though they will likely "fill in the blanks" with some sort of guess (often, a mental health break) -- so if your truth is more favorable to you than their guess, you may want to be forthcoming.

Agreed. And the best bulwark against any suspicions about those "blanks" that get filled in will be continued academic excellence.
