As long as you didn't have any other relationship with them, I don't see any multiple relationship issue. I would though, wonder about returning for treatment depending on what goals were not met the last time. Granted I come from a very goals-focused framework. Maybe they need something else?

Unless you referred them to yourself, I don't know that there are really any ethical considerations, especially if this is a person with whom you initially terminated, and they're now returning for additional/new treatment.
