When people talk about applying "late" being the cause of rejections, are they referring to really close to the deadline, or does this encompass all those who take the august mcat?

That usually means people who apply through AMCAS after getting their August MCAT scores sometime in October, not those who applied in August or September, are processed, who are just waiting for their scores. Although schools won't finalize your application until they see the MCAT, they will still send out secondaries for you to complete. Just make sure you have the secondaries completed by the time your MCAT score comes out and you should be fine. In this case, you would probably be somewhere around the middle of the pack.
All depends on what kind of admissions process your school uses. The later you apply with rolling admissions the fewer class positions you're competing for, so the harder it is to get in.
