I am an ORM F with mid to high stats and good EC’s with a well-rounded story on significant family with chronic disease and how that led me to have a specific interest in disease management and underserved communities. Many of these schools have MCAT averages 2-3 points below mine. What if I applied to approximately 20 of these and another 10 schools with slightly higher averages, but are more service and community oriented?

Yes, but avoid most OOS state public schools.
