You may look a bit complicated to AdComs. On your side are some good ECs (you played soccer for Canada or one of your other heritages?). Against you is a complete lack of clinical experience so far. Complicating matters is your Canadian nationality, which possibly makes your URM status less of a draw because it does not connect to underserved populations in the USA.

I would suggest that you need substantive clinical experience, and that this should preferably be in the USA. If you can connect clinical experience in the USA to your URM status that could help. Your GPA is low for an international applicant, which suggests that DO might be your more successful route. A great MCAT seems essential.
