What exactly does one do who is certified in wilderness medicine? Is there pay for this or is more volunteer search/rescue type stuff?

In most cases, "wilderness medicine" is practically synonymous with "first aid."

Don't count on making a specialty out of it.

There is a wilderness medicine subculture.... with quite a few certification courses out there (most are basically first aid like Bluedog said). It can get a little more complicated on the MD side of things, but it's a hobby, not a way to make a living.
