Where is this MD/PhD recommendation form that they speak of? I cannot find it anywhere.

Also, is anyone else getting confused about the letters of recommendation? I have five letters that are coming in a committee packet plus one that is coming separately. On the application, though, it says to submit the committee packet IN ADDITION to the three science references they ask for. I'm not going to be at a disadvantage because some of my references are part of the committee letter, right?

I called and asked about the LORs a few weeks ago; the listing of letters is just for them to figure out what mail to acknowledge. So indicate your committee packet and the separate letter. I'd try to have three recommenders fill out the MD/PhD recommendation form, even if these are submitted under separate cover.

A: The MD/PhD Recommendation Form was developed by the MD/PhD Admissions Committee to help answer questions that may not be included in your letter of recommendation. Your application will be completed with or without this form; however,
it is to the applicant’s advantage to have this form sent, even if it is sent under separate cover.
 
my recommenders did not use this form (I use VirtualEvals), and i recently received an interview invite, so I wouldn't sweat it too much. then again, that FAQ is pretty to-the-point, so i dunno
