Just thought I'd say that the Columbia secondary for this year has barely changed at all, it looks like only some of the character limits were changed slightly. Here's the questions (basically the same as the ones on the first page):

6. If your education has been interrupted for any reason, please indicate briefly the reasons, the duration of the interruption and how your time was spent.
