Try a plain old #2 pencil.
Staedtler make universal pens with alcohol based inks that are water fast. The ink is removable by isopropyl alcohol but you may want to double check on an extracted tooth or a sample surface of what you want to mark. http://www.staedtler.com/en/product...-permanent-pen-313-permanent-universal-pen-s/
Try a plain old #2 pencil.
This is what I've seen OMS use for implants/etc, it can even go through the autoclave as well apparently. They used the small ones w/o erasers like for golf.
pencil is good for teeth, but hard to mark on gingiva. I'd say Thompson stick is my #1 (cheap), and then sharpie. Here's a photo of markings i made prior to EXT/implant placement
