Doctors went to medical school too. They've been through the same things you're struggling with right now. They know the process. You'd be surprised how understanding your preceptors are if you actually try to communicate with them. This should not be a question you have to ask on SDN.
Probably depends on the attending. There are going to be sticks in the mud who take offense to you not prioritizing their clinic over everything else. Most people should understand, unless you asked to leave at like 8:30 AM. You should have a feel for your preceptor by now.
What do you mean by "early?" If you mean to leave with enough time to get to you exam, that's obviously fine. If you mean "leave at noon so you can cram a few hours before your exam," that's probably not gonna fly.
What I would do is probably give your preceptor a heads up a few days in advance of your exam (maybe slip it into regular conversation). They'll then know why you may be "stressed" on the day of the exam. They may even independently offer to let you go early to prepare for the exam. But I would not explicitly ask for (or expect) any more time other than the amount you need to get to your exam comfortably.

I’m assuming this is preclinical since you said midterm.
Why on earth do schools schedule “mandatory clinical experience events” and such right before big exams? Always annoying imo

Just ask! I always let my students leave early if they have other school commitments, tests, interviews, etc.
Like DrStephenStrange said, communicate with your attending. Great way to build interpersonal skills.
