The Pre-Pharm area has a lot more information for you on the PCAT, but to answer your questions:
1. You probably won't be asked anything past the first 2 rows so I would focus on these...H, He, C, N, O, and F comes up an awful lot in just about any class...maybe Na, P, S, and Cl too just to be on the safe side. I can't remember exactly what they asked but I'm pretty sure that they gave the relevent info for the elements when they asked questions but it won't hurt you to know these for pharmschool anyways.
2. The only advice I can give other than to get a couple practice books and try your best to learn the basics. It really doesn't get too indepth but the test covers a lot of area so you need to know a little about a lot of things, not a lot about a few things. The most important advice I can give is to do a lot of timed practices on the math...no one ever seems to have enough time to finish even though the problems are pretty simple. My mistake on this part was realizing I knew I could solve the problems but then ignoring how long it would take me to solve them. I had to christmas tree about 20 of them because the time ran out.
