I'm pretty down about my Step I score, I scored a 210, about 20 below my target score and what I had been getting on my practice exams. I wanted to do IM or peds at a larger university program in the NE (considering h/o). Will this keep me out of middle tier programs (more established programs Jefferson, Temple, Brown)? I know that there is more to one's application than Step I but it's a fact that standardized tests are the easiest way to screen out applicants.

It will probably be just fine. If you're worried about it, focus your efforts on doing well third year; make the rest of your application shine.

But medicine and peds are huge fields, so I'd bet you'll find a match you're happy with as long as you're not unreasonably picky.

Yeah don't be too restrictive about where you apply, and also make sure you bolster your apps with good clinical grades. Away rotations at those sites helps.
