Most programs seem to ask for 3 LORs. I have two from my 3rd year rotations and one from the chair. I asked my sub-I attending to write me one too but he's slow about doing so and I'm worried it won't be in on time. Is it ok if I don't have a Sub-I letter from the chair in?

Will it be okay with just 2 3rd years and the chair?

Sure...you can still have the letter from the Sub-I chair submitted when he gets around to writing it (provided that's before November 1) and uploaded to ERAS.

Have you inquired as to whether there is anything you can do to help him get the letter done (ie, what's taking him so long)?
 
On that topic, I assume that one letter comes from your surgery clinical faculty, and one from a sub-I/fourth year surgical elective faculty, does it matter where the third comes from? Should it be from another surgeon, or someone from a different specialty? Thanks!

Surgeon probably best but if you have another from a different specialty it should probably be:

- surgical subspecialty
- critical care
- gastroenterology
