How important are 3rd year grades in ophthalmology compared to other specialties like IM?

Does H=honors? Does your school have high honors? The fact that this is so imbiguous is telling. Having looked at quite a few applications, it's actually very difficult to compare grades among different schools even if they do provide distribution data. Frankly, it's too time consuming to look this over in great detail. Certanly a gestalt suggesting strong academic performance is very important, but the details aren't something to freak out about. It's actually much easier (for better or worse) to compare step 1&2 scores, and they may - rightly or wrongly - be use to help to sort out which places have grade inflation and which do not.

Strong letters of recommendation from ophtho faculty are super important. These are probably the only letters that will be read thoroughly. By MSPE you mean Dean's Letter? My feeling is that most people skim these to mine them for interesting biographical details, and then flip to the summative paragraph where your program states, often using code words, how strong an applicant they feel you are.
