Currently a pgy3. I’m interested in neurohospitalist 7on 7off. Looking at jobs online, some don’t mention if it’s 7on day shifts or not. Does that mean that during the 7on you are on for 24 hours? And if other jobs say you would be on call 1:4, that means you would be on call even during the off week correct?
For a 7on day shifts, do you actually finish at 7pm or do you have to stay extra to finish notes, etc. thank you.

The jobs vary considerably. I currently work at a hospital 7on/7off. When I’m on, I’m on 24 hrs x 7 days. The reason I’m able to do that is because acute stroke is handled by tele-neurology (day and night). My previous job was essentially q2-q3 and I had to go in all hours of the day and night for acute stroke, seizures, and even LPs. It was hell on earth. I do locums at a bigger hospital M-F from 8-5 where I see acute stroke and frequent ridiculous “STAT” consults. It works out because I’m completely off at night. Just remember that these hospitals will take advantage of you if you let them. They are oriented at making the most money and could care less about your quality of life.

Doc's in our practice have found that the 7/7 model isn't as lifestyle friendly as it appears. Usually the hospitals do want 24h x 7 days on, then 7 days off. I think its a passing fad.

7 on/off jobs are variable in terms of intensity. You have to see how many beds the hospitals has? For instance, you will be very busy if you are the only consulting neurologist covering a +500 bed hospital. You have to see if the hospital has tele-stroke set up? If not, they will be calling you for every acute stroke rule out 24/7.

As mentioned above, its variable and depends on many factors. One of which is how efficient you yourself are.
Some are in bigger, busier hospital and you end up working 10-14 hours a day. For these very busy hospitals with >10-15 new consults a day, 12 hour shifts are usually the norm. For smaller and less busy hospitals you will probably be on call for 24 hours and see 5-10 new consults/day and could get done in 6-8 hours. There are some sweet gigs in smaller hospitals or VA hospitals where you might have 2-4 new consults only and work 4-6 hours a day and get couple of calls after hours.
