I served at a hospital in Kapsowar, Africa about a year ago for a month and a half
Unfortunately, the doctor who used to serve there no longer works there

I was wondering how I can go about this? Perhaps put contact information on my application?

But most of all, do admissions check to confirm your ECs??

I would just put what you can. I think for my trip, I listed the trip coordinator, not someone from where I volunteered.

They generally don't check up on these things - it would take way too much time to check out all of their applicants ECs!

They probably won't personally call the doc.
I agree w/above.
Sometimes they check things...I know one of the hospitals where I volunteered got a call from an adcom @my state med school. They definitely don't check everything.
Just don't lie about anything.
