AADSAS Transcript form

I was wondering if anyone could post the form that you are supposed to attach with your transcripts when sending them to AADSAS, reason being I want to give it to the registar office before I leave school, which will be before the opening of AADSAS. I just don't want to have to go through the hassle of them possibly losing it or not attaching it if i have to send it to them via fax or mail.

You have to open an account so you will be given an ID number. After you've opened your account, you can print out the form. The form will have your ID number on it. I think your transcripts can only be sent with that form with your given number. Hope this info helps...

Here is a link to a previous thread about this same question. The form is downloadable there. But you will need your AADSAS ID number as stated in the previous post.

yea i know that i need my AADSAS number in order for it to be valid, but i just wanted to give the registrar office the form, and i was goign to call them with the AADSAS number when i opened my account, the link that was provided was for the recommendation letter form, which i already have. I am lookng for the transcript form that you attach to your transcripts.

I have heard both yes and no on this site regarding my including the form in one big envelope with sealed transcripts that I have had my schools send to me. Anyone try this and have any problems?
