I failed STEP 1 and I previously wanted to do surgery or a surgical subspecialty at an academic institution. I know now that that is much harder to attain, and I am between continuing that dream or applying to more realistic specialties that I am much less passionate about like EM, Peds, Community IM. I'm feeling very lost and I was wondering if anyone has any advice or success stories.

I understand the first response is to start thinking about the effects on long term things. I'd say to first focus on passing Step 1 (if you haven't already). Next, study hard for Step 2 and see how you do. Once you have a Step 2 score, I think you'll be able to more accurately decide what specialties to target in light of Step 1.
