academic medicine and underserved communities

I just have a quick quesiton.
So I would like to work in underserved communities, but I would also like to work in an academic based medicine group (like residency training, attending physician, etc). Does this sound conflicting? I am asking because a secondary question has asked me to note this.

Also, maybe I am misunderstanding what academic based medicine is.

Nope. Plenty of universities are affiliated with county hospitals.

Yep! At the hospital where I work, all the doctors are employees of both the university and the county hospital. A lot of them divide their time between clinical medicine and research.

There also exist many academic medical centers in underserved communities (CUMC, USC-Keck, EVMS, etc)
