If you think that intramural sports helped you develop qualities to become an optometrist then yes. For me I considered it almost like a job, you want to show the optometry schools your applying to your academic background, as well as your professional experience (shadowing, jobs, volunteering, clubs, etc.) I played D3 college soccer, and was actually a captain last year, I didn't put it on my resume. Best things to put are there is the stuff that shows your character, academic "good-standing," and your experience in the field or your major.
