Hi everyone,

I am looking to start a residency in the US this year and I was hoping to find some options for ROS in academics. Since I am interested in the academic career track (clinician scientist), I would hate to have that 2 yr hiatus when I am fresh out of residency+fellowship towards ROS if its not in academics. Is it impossible to imagine that places like UToronto, McMaster would hire people for their return of service? Can you work with past mentors at these univ in research or clinical or both capacities and have that accounted for as ROS?

If the above is not possible, is there ANY way to avoid the ROS in Canada and do a waiver in the US? My last conversation with health canada sounded pretty absolute in the sense that you have to come back but then again I know of some examples who continued on in ths US. A waiver seems more appealing than ROS since I know of a few people continuing with the research they started in their fellowship while doing a waiver at a VA of some major univ.

I hope to hear from people who have trained in the US and have come back to academics in Canada or anyone intending to do so. Thanks 🙂

What ROS are you referring to? There is no ROS in Canada if you have done residency in the US. If you are thinking of the J-1 visa two year return to home country regulation, this is a US immigration requirement, not a Canadian one.

Waivers are given to doctors who were on J-1 Visas if they are willing to work in primary care for 3 to 5 years in an underserved region in the US. VA centers can recommend waivers for doctors who have been heavily involved in research that is vital to the US. You might qualify for an O visa if you are well published.

Forgive me for partially hijacking this thread, but does the requirement of 2 years back in Canada after completing graduate medical education in the US on a J-1 visa require the physician to practice in a rural/underserved area or can he/she practice in Vancouver, Calgary, Toronto, Montreal, etc.? Thanks!

NO. During those two year back in your home country you can work anywhere you want (as long as you get hired). There are no limitations on where you can be employed as long as you meet the licensing requirements for that province. The only way you will be limited to where you can work is if you do residency in Canada as an IMG and have to sign a provincial return of service. But you will most likely be on a temp license and have to be under supervision.

To the OP who is considering research, you can do a post doc fellowship anywhere in Canada for 2 years before flying back to the USA for an academic position. Also as NewCanadian said you can get an O1 to defer the 2 year residency requirement, HOWEVER, the 2 years must be done before you can apply for green card or H1b visa or NAFTA visa etc... Either way you will have to apply for exemption/waiver or return to Canada for 2 years. Even if you marry an American citizen after starting your J1 visa, you still have to return to Canada for 2 years.
