Academic vs. Community?

As I'm agonizing over my rank list like everyone else, I'm trying to decide where to rank the community program I interviewed at. I really liked it, but am a little iffy about going to a community program, especially because I'm not totally sure what I want to do in 4 years. Any experiences from the residents, especially from those in community programs, would be much appreciated. Thanks!

What specifically are you looking for with respect to our experiences at community programs?

In general I love my program and the training I've got. We serve an inner city population, so there is a "county hospital" kinda feel to it, but that has equaled more numbers. Its a 20 resident program with a good balance in training, and helped me along to a fellowship. We have a strict didactics schedule, simulation center, and board pass rate which rivals the "acedemic" programs in the region.

All community programs are not equal, same as all academic programs are not equal. I am a strong proponent of what I call "academic community programs". These are the programs with strong university affiliations, strong research (and research requirements), a big referral center with all subspecialities represented. Basically, an academic program without the fellows or the attached university. In these programs you typically get the numbers associated with the community programs and the teaching of the academic places.

I love my program, and I am sure the training rivals anywhere in the country. Lots of diamonds in the rough out there, if you find one, grab on!

I am at one of those "academic community programs" also, and we get tons of MFM referrals, great Uro-Gyn, more Onc than I want (ugh), plus all the perks of a community hospital (really great nurses, lots of exposure to private physicians and we get to do their gyn cases, no fellows around to steal your stuff!). I was really biased against community programs at first, but the interview here in Asheville really perked my interest, and I don't regret my choice for a minute!

I'm leaning towards a community program but I'm looking for something with a good volume that's not too high but not so low that you're twiddling your thumbs. Any ideas?

I'm also looking for 'academic community programs' that have a decent fellowship placement. Mainly along the east coast, west coast, Texas, and states around the great lakes. any thoughts would be much appreciated!

My first two thoughts are Cedars-Sinai Medical Center in LA and Baylor University Medical Center in Dallas. Though very different, they are the 1st and 2nd largest hospitals in the country, respectively. Both programs, especially Cedars, have sent residents to various fellowships around the country.
