The fellowship application season is upon us. Would anybody please comment on ACGME-accredited Procedural Dermatology fellowships vs ACMS fellowships? Do you think Mohs will ever be a part of ABMS?

1. Many of the the good ones are accredited by both, but going forward, the ACGME is the only one that really matters.

2. Mohs will probably never be part of ABMS. There's too much natural opposition:
-- Dermatologists that are not fellowship-trained, but want to do Mohs will be against it, because then they will be excluded
-- Other surgeons have a huge problem with any dermatologist being referred to as a surgeon in any capacity.
-- To get over this, the designation "procedural dermatology" was introduced, but that had the effect of potentially excluding even more dermatologists without fellowship training who do some or all of the procedures encompassed by the term procedural dermatology.

There are other reasons as well.

I would give the PD programs a higher rank on the ROL given that the Mohs College has granted them "deemed" status (there's no downside).

Sadly, no, I do not believe PD or Mohs will be a designated specialty. The AAD will never sign on for that -- far too many people making $$$ without putting in the training time for that to happen.
