Adding letters of recommendation

Hi, I had two questions about adding additional letters of recommendation after submitting the primary application

Does this apply to creating a totally new letter and delivering it to AMCAS, or only to delivering a letter you already created the entry for?

Also, can you add a letter that you already had delivered prior to submitting the primary, to a school that you initially declined to send it to?
