Try calling the ADCOM office. I don't have it off hand, but I think you send it in with your secondary.

I called a few schools the other day and it seems like Temple had a recording that said their secondary app wouldn't even be available till Aug. 1. There's a chance I could be remembering wrong, but go ahead and give them a call.
