Ok, I finally settled on my rank order. I now have programs 1-14 listed in the correct order. I am applying to Rad Onc, so I also have the appropriate prelim years assigned on different supplemental lists. However, I am still confused on one point...how do I place a prelim year in a "back up" spot in case I don't match into Rad Onc at all? Do I just put a prelim year in spot 15 on the main list? If so, will I then be given the "Congratulations, You've matched." message on March 13 only to find out on Match day that I didn't rank in rad onc...just a medicine internship? 🙁

Thanks for the help!

If there are 1+3 or 1+4 programs on your rank list (rads,rad-onc, ER, etc) you will be notified if you don't match into an advanced position, even if you match into a PGY1 spot. So rank your prelim or transitional programs 15-20 or whatever. You'll still have access to the scramble, and will be notified the same way as unmatched categoricals (except that you will already have a PGY1 spot). Good Luck!

Just wondering if the converse (inverse?) ever happens? In other words, one matches into an advanced position but does not get a preliminary or transitional spot. Is that possible?

Yes it is possible to match into an advanced position and not a prelim spot. You will then get a message from the NRMP saying, "Congratulations, you matched into an advanced position!" You then have to scramble for a prelim spot. Not the best situation to be in, but it sure as hell beats not matching into your advanced position.
