Advice/help with a Recommendation

I am a high school senior applying to a dual dental program. I have a dental internship and have asked my mentor for a recommendation but because this is his first experience at having an intern, etc., etc., he is having a hard time wording the rec. We have spoken many times about the dental field, why I want to become a dentist, I have given him a resume, he gives me high grades each semester, but he has no idea on how to word the rec. I don't think he is reluctant to give a rec, I just find that it's been a learning experience for him and I in allowing me this opportunity to intern at his office and that along with that came responsibilities like filling out forms on my performance and reporting back to my school in a timely manner. Can anyone please help me help him help me?? If you need to please send me a private message. Thanks.
 
A lor can take on many formats but good ones generally address the following in some manner: firstly, how the writer knows you, your dedication to whatever you are doing, your character traits, how these traits will be an asset for a dental career.
