I know it's late in the game.

I have had one interview this cycle after applying to 40 schools.

1) any advice on the outlook and when to prepare for another cycle?
2) anything I could do to improve the chances with the 18 schools I haven't heard from?
 
To improve your chances start researching schools that respond positively to update letters. That information should be available somewhere here, I know UVM does. As for anything else it's hard to say without knowing your stats/ when you were complete at schools

Thanks for the reply- stats are 3.7/3.3 gpa 31 mcat balanced. I was complete at most schools by the end of August and some mid September.
 
1) The game isn't over until it's over. This is traditionally somewhat of a lull period in admissions as most or all acceptances have gone out at this point, making the class lists "saturated." Things will start speed up once people begin making their decisions and withdrawing from schools. Revisit weekends will also be happening over the next month or so, which again supports movement on the class rosters. If you haven't received an acceptance by May 15 - even though you may be on multiple waitlists - I would begin working on your application for your next cycle. But don't simply fill in the application again with the same information. Make sure that you've actually taken some time to reflect and make changes to improve your app. If you don't do that, don't expect anything different next year.

2) As said above, just about the best thing you can do at this point is send update letters to any schools you interviewed at (if they accept them). Tell them about what you've been up to, your fall grades (assuming they're good), any extracurricular or academic achievements, and maybe a line or two emphasizing your interest in the school and why you want to be a student there. Other than that, there's not much to do other than wait.
 
Like NickNaylor said, unless you've done some significant things to change your app within this past application cycle, in the case you have to reapply, you should wait another year to change few things in your app. In the meantime, although some schools are done with interviews and others are still winding down on their interview season, there are still a couple IIs going out. Send in update letters to show that you're still interested in interviewing.
 
Were you rejected after your one interview or are you on the WL? You probably Need to start buffing up your application now so you can reapply early. What have you done this year to improve your application? ADCOMs will want to see improvement over your last application.

Haven't been rejected or WL yet- I will find that out mid-march

Did you send secondaries to all 40 schools?

If you did, I would recommend reducing the number of schools you applied to. I only suggest this because you may have compromised secondary application quality for quantity. I can imagine writing secondary essays for 40 schools is daunting, and it would be easy to just recycle essays or give generic essays.

Don't get too worried yet, you still have time. However, you should start working on building your application up. Schools will want to see continued activity over the previous year.
