After your first year of internship what are your options if you are not in a civilian residency? The options that I know so far are: 1. GMO, 2. Undersea Medicine, 3. Flight Surgery. Do all of these require Step 3 to be completed first? Are there any others?
 
You left off the biggest one: 4. Go on to (military) residency. I'm fairly certain that you have to have a state license to do any GMO tour, including DM and FS, which means you have to complete step 3 and at least one year of GME.

Options 1-3 are the same basic thing. What's your branch, and is your internship a categorical one? Navy sends almost everyone on GMO tour after internship, as I understand it. I believe there was an AF memo last year that said that everyone not matching into a categorical internship would do a FS tour before being allowed to complete a residency (not sure on the validity of this). I'm told the Army generally only sends people to GMO/FS tours if they fail to match into a residency program (but that they use board-certified physicians in GMO slots to make up the difference).
