AI scribe?

This forum made possible through the generous support of SDN members, donors, and sponsors. Thank you.
DAX is a Nuance/Dragon product but I think they have integration deals with several EMRs. My partner uses a non-integrated version of it with our EMR and loves it. I’m using Insight and just copy/pasting it over and love it.

I haven’t tried Dax, but I’ve heard good things. I’ve played around with Insight and it’s ok for the HPI portion but beyond that I didn’t find it very useful. I don’t like waiting around for it to generate the note and then because there is no integration you have to find a way to copy and paste it into the EMR. I used the free version so I didn’t have any custom templates which I’m sure would be helpful but as of now it’s faster for me to use templates/smart phrases and dragon dictation.

Are you guys using ai for physical exam/assessment/plan or just the HPI portion. Would love to cut down on time spent doing notes but I’m pretty picky

@drusso @callmeanesthesia what do you use to record the conversation?
The Insight web app records the conversation and outputs the note. I use an iPad for that. Then I also have the site logged in on the exam room computer. I review the output and make edits, then my scribe copies and pastes the outputs into the relevant sections.

That doesn’t sound truly autonomous and if you have to go back and pick and choose pasting of the output if you’re solo or have a scribe do it, seems to defeat some of the purpose 🤷🏻‍♂️. I tried the free version and it didn’t populate my templates well. It also absolutely annihilated battery on my phone. So was curious if people have gotten it to work better than I did.

I tried DAX copilot briefly for Epic. It was longer to explain to the patient, log in, and make the necessary edits. My notes are fairly short and the AI generated ones have a lot of fluff. Followups I copy and paste 70% of my note anyways. I have 2-3 lines to hpi, minor edit to the PE that was copy/paste, and 2 lines for plan.

I like it for new patients especially. Something like an MBB f/u I don’t bother. My MAs serve as scribes (and procedure room techs) so it’s not really an extra employee with my workflow. That said, I’ve liked it a lot because it does a generally good job of picking up details. It takes me maybe 1 minute to proofread and edit the output. Then I click copy all, then paste it into the HPI, delete the header and physical exam section (usually my scribe has entered that as I’m doing it, so I don’t have to speak all my findings aloud to the device), then cut the assessment and plan and paste it into that section. Probably 2-3 minutes total depending on how much editing I had to do, and the note is done.

What emr do you use?

If I can't be completely done with the note after leaving the exam room these options don't sound that helpful. I'll just dictate as I normally do and be done with the note instead of proofing something later and copy pasting into various sections of the note.

I proofread and copy/paste it in before leaving the room. Takes about as long as dictating except I don’t have to come back and proofread and sign the dictation later (a cumbersome process in our EMR). The note is done and charges submitted before I leave the room. I use a separate iPad that I keep plugged in to avoid draining the battery on my phone.
