This is a weird year and as I get ready for 4th year I am becoming slightly nervous.

MD student at low tier state school interested in pulm/crit.
step 1: 230-235
Step 2: pending but shooting for 245+
mostly HP so far in ms3 with excellent feedback on rotations.
Research: 3 papers pending pub. Multiple posters presentations. Some minor awards for presentations.
pretty average EC’s but did get awarded for volunteer work.

I am having a difficult time gauging the true competitiveness of academic programs given the vast diversity in IM. Any help is super appreciated!

There are low/mid/upper tier academic programs. You are solidly competitive for low/mid assuming you are a US student.

I am a US student. Can you give me an idea of what I low to mid tier program might look like?
You can use bottom 30-40 US meds schools that have an IM program as a surrogate for low tier. These programs for the most part have minimal research opportunities; they are not affiliated with major hospital systems; fellowship opportunities are limited (e.g. 2-3 spots per year in cardio, GI, Heme/onc ect). Ex: Florida State University, Louisiana State, West Virginia etc...

Mid tiers have research opportunities; affiliated with major hospital systems, and have abundance of fellowship opportunities. Ex: University of Miami, University of Florida etc..

Top tiers have everything mid tiers have plus the brand name: Ex: Beth Israel, John Hopkins, Duke, UPitts, UPenn etc...

These are general rules but there are exceptions.
