AMCAS Repeated courses?

I retook Intro chemistry later in my undergraduate (C -> A) and Calculus II (D -> A+).

My school does grade replacement when you you receive a C- and lower, but no replacement if you achieved a C and above. So my original D in calculus is 0 credits and my A in calculus is 3 credits, but my
A in Chem (retake) is 0 credits (earned).

So for my AMCAS, do I list the retakes respectively in what they match on my transcript (0 and 3), or do I count both credits attempted (3 and 3)?

Also when stating you retook a course, do you select repeat for both the 1st and 2nd attempt?
I tried searching this in the forums, but got conflicting replies.
