For contact's title, what did you guys use for a research project in which the contact was a doctor? I think plain old 'Doctor' should do.

Someone P. Somebody, MD, PhD, FACP, etc, etc.

Pretty sure that you don't need a prefix if you use the suffix, and I'm pretty sure you're supposed to use the suffix in official documentation because it's more accurate when talking about credentials (as opposed to putting Doctor when that could be PhD, MD, DO, DNP, DDS, etc).
