Does anyone where knows any programmes that have this kind of combination? perhaps with a following CCM fellowship...

Hmmm ... I don't know of an Anesthesia/IM program, but the ABA and ABP have just recently established the combined 5-year Anesthesiology/Pediatrics residency. My guess is it's for people who want to specialize in the perioperative and critical management of the pediatric surgical patient.
