Answering every question correctly = 45?

Are you guaranteed a 45 on every MCAT test date if you manage to answer every question correctly?

Yes, because if it is not a 45, what else would you have to do to get a 45? Finish the exam with no breaks perhaps.

It's actually a good question because if they make a section extremely easy and too many people get all the questions right, they cannot give them all 15s because its supposed to represent the 99.9% percentile. It is VERY unlikely that that too many people get 100% on even a section, but I'm sure it has happened at some point and they were left scratching their heads for that month of scale determination.
