Antiretroviral dilemma

The other day a presumed HIV positive patient came to pick up their antiretroviral meds, but unfortunately had lost Medicaid coverage. One of the Rxs was extremely expensive.😱

Since the patient could not afford one, she asked if she could pick up only the other one.
I was under the impression that you have to take the whole cocktail or there is a risk of developing quick resistance.

Has anybody run into this problem, and how have you handled it?

my pharmacy is smack in the middle of progressively gentrified area so we have financially well off patients and those who could never afford their meds without medicaid. we have had cases when insurance didnt cover one of the antiretrovirals and the guy paid the full amount and later got reimbursed. in another instance, we gave a three day supply until the insurance could cover again.
