No, of course the slate is not wiped clean. However, there are some fellowship programs that don't require you to send med school transcripts, so theoretically they may not notice that it took you so long to graduate. They will notice the failed steps, however, most likely. A few schools don't require that you send USMLE scores, but in my experience those few required med school transcripts.
If you can prove or at least give evidence that the brain tumor was what caused your failures on Step 1, that might change things some.
The step 2 score is also low for an applicant to a competitive IM specialty, but again, some program directors don't seem to care much about step scores.
At this point I think that if you like IM, I think that you should just plan to forge ahead and get into the best IM program that you can find, that you think you will learn well in and that will take you. The other stuff, like picking a subspecialty, can wait until later.
