Anyone know how retake classes are calculated?

I know there are some PT programs that simply take the higher grade for a class that has been retaken.
There are others that average the two grades. For example, I received a C (2.0 QP) in one of my classes the first time around and then got a B (3.0 QP) the second time around. To calculate the grade for that class, do PT schools take the average of those two (2.5) and multiply it by the number of credits the class was?
 
I'm also in the same situation - C- to B+ grade. The schools that average the grade will just use the calculations made by PTCAS, but the ones that take the highest will calculate your GPA for you. They calculate it just how you normally do, but use the higher grade instead of the lower, and PTCAS will include the lower grade in the calculations, as though it's a completely separate pre-req class.
