Anyone still billing for three level RFA?

As the title says, is anyone still billing a three level RFA? I routinely do a L3/4 and L5/S1 RFA and eat the L4/5.
However, that's a free needle that costs me money in overhead.

Also, can you do a L4/5 and L5/S1 RFA and then come back later to do a L2/3 and L3/4? and 6-8 months later repeat it again if necessary? or does that go against the 2 session per year

I can still bill 3 level RFA for state WC, Cigna, Aetna, UHC, auto.

I can’t bill 3 level RFA for any federal insurance—mcr, mcr advantage, Medicaid, tricare, or for BCBS plans.
