honors=a
high pass=b
pass=c

Basically. I think that many medschools use to H/HP/P system to avoid the stigma of "B" and "C" grades. So many people in medschool have never made a C in their lives, and would probably flip out if they saw several Cs on their transcript, while a "Pass" might not be as stressful? In talking to a few people at several medschools, though, grades at A/B/C places might be slightly more inflated. (Of course there are exceptions). At two different ABC schools I know of, you really have to do poorly to get a C, while at H/HP/P places, P's are slightly more common place. All that said, my sample size is 2 ABC schools, and 4 H/HP/P schools.
 
Honors == No more than top 15% of class, usually around top 12% gradewise
High Pass == individuals that don't get honors, but > top 50%
Pass == Bottom 50% of class

At my school it was divided 30/30/40 for H/HP/P. Its really variable.

My school's about:
15-20% Honors
15-20% HP
60-70% Pass
 
where i go to school:
5-10% honors
rest are pass/fail
we don't have a high pass distinction; also to honor any rotation, you have to get above the 90th percentile on the subject's national board exam.

Can you imagine how much more pressure that would put on your USMLE? Besides I think most PDs at least glance at the histogram on the back of the Dean's letter to get an overall impression of how much a schools grades are inflated/deflated. But then again, some surgery programs do screen general surgical applicants by if they got Honors in Surgery.
