Application Station- Certiphi screening

For anyone that has completed their application station form, I have a quick question. On the page titled "additional information", look at the 2 'yes and no' boxes, they're not aligned, I can't tell which box corresponds to the 'yes' and which corresponds to the 'no'. Its really weird, can anyone lend their input

The bottom one is no...or it was for me anyways. Just click one and don't enter anything in the text boxes...if you selected the "No" check boxes correctly then when you click next the application will continue on the correct page. If you did it wrong then it will say you need to fill out and explain why you checked yes. I hope that makes sense..

when is this thing due?

Might be different for other school but, "Candidates planning to matriculate at the UB School of Dental Medicine must initiate their background check no later than the deadline date to accept their offer of admission."
 
Ok, thanks. Greatful to get to this step, but jeez. 1k, then 150$, what else is next?
 
Hey guys, so I completed and submitted the Certiphi application yesterday, but then I received an e-mail from them this morning asking me to call and confirm my security number. So I called and re-confirmed my SSN over the phone, and now they're asking me to send a scanned image of my social security card...!!!

Should I be worried or is this standard procedure? Has this happened to anyone else?
