I was shadowing an internist today, and he told me that when you apply for a residency, you will be selected based on high school, college and medical school records and activities. But I have also heard that only your most current academic work is looked at. Which is true?

I think your med school grades, particularly the last two years, and activities matter. I believe your performance in electives related to the residency you desire matter a great deal. You should probably ask the question in the allopathic forum, you'll likely get a more informed and accurate answer.
